As a Reablement Team Leader, you’ll support adults across Hampshire who need extra care and assistance. Our Community Response Service focuses on short-term assessment and reablement for those with various short and long-term conditions and frailties, including those transitioning from hospital care. We are dedicated to promoting independence and enhancing the quality of life for our service users, helping them stay at home longer. What you’ll do: Triage and Assessment: Collaborate with health colleagues to receive and assess referrals, using a strengths-based approach to identify needs and set goals. Coordination and Care: Coordinate care to promote daily living skills and independence, working with partners like the Frailty Support Service and hospitals. Team Leadership: Oversee a team of Community Reablement Assistants, providing motivation, coaching, and ensuring high-quality service delivery. What we’re looking for: Relevant experience: With significant experience in a social or health care setting, you’ll have a proven track record of successfully leading, motivating and developing others. Knowledge: A good understanding of the needs of the service users and the impact of this on individuals, their carers and relatives. You’ll know what makes good care practice and how to deliver social care reablement.
