Job summary
The post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme.
We have a vacant 12 month fixed term Band 5 Community Nurse/ Care Co-ordinator post to fill.
The service runs 8-8 every day seven days a week with the expectation to cover some 8-8 Monday through to Sunday.
Main duties of the job
An exciting opportunity to join a highly experienced multi-disciplinary team within the Teesside locality working and supporting people with Learning Disabilities to achieve high quality of life and optimum health. The role is based within Middlesbrough, Redcar and Cleveland, based at Flatts Lane Centre Normanby .
The successful candidates will carry and manage a case load.
You will need excellent communication skills and be calm, confident and resilient in your approach. You will need to be able to form therapeutic relationships with people with learning disabilities their families and carers.

1. Current professional registration with Nursing & Midwifery Council RNMH or RNLD
2. To be recognised practice assessor / mentor or work towards in an agreed timescale
3. Trained Clinical Supervisor (within 6 month probationary period)
4. Evidence of Continuing Professional Development
Experience

5. Working with people with mental ill health / learning disabilities
6. Working in a person centred therapeutic organisation
7. Working collaboratively with service users with a learning disability and their families/carers
