Job Summary
The successful candidate will be a registered nurse with substantial experience in cardiology nursing, knowledge of cardiovascular disease and treatments, excellent communication skills, and the ability to work autonomously or as part of the cardiac rehab team.
Main duties of the job
* Deliver cardiac rehabilitation and heart failure care primarily at DMH, with potential travel to other sites.
* Provide phase I care and collaborate with the cardiac rehab team for high-quality patient care.
* Conduct post-MI reviews, assist in loop implants and cardioversions.

Job Responsibilities
* Manage patients with cardiac diagnoses independently, reporting to specialist nurses or the multidisciplinary team.
* Develop evidence-based guidelines and individualised care plans.
* Coordinate with primary and secondary care teams and document changes promptly.
* Provide training on CHD and heart failure management.
* Assist patients in risk management and self-care behavioral changes.
* Monitor and adjust medications within protocols.
