Chorley Central PCN are recruiting a Clinical Pharmacist to support patient care in line with the Network Contract. The working days are to be agreed but the post will be 24 hours.
This is an excellent opportunity to join our existing team of 5 Pharmacists and 3 Pharmacy Technicians and work as part of our well-established Network.
Main duties of the job
The successful candidate will provide clinical support on medicines optimisation and quality improvement.
The post holder will be responsible for reconciling clinical discharge summaries and, where appropriate, communicating changes to patients.

Clinical Pharmacist Job Description
The post holder will provide primary support to general practice staff regarding prescription and medication queries. They will help support the repeat prescriptions system.
The post holder will work to integrate practices with community and hospital pharmacies to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies, requiring motivation and passion to deliver excellent service within general practice.
Reconciliation of medications on discharge from hospital and other care facilities, implementing any monitoring or dose titration required, including arranging blood tests and clinical review if needed. Identify high-risk medications from discharge and ensure monitoring and prescribing are safe and consistent with Local Medicines Management Guidance. Pro-actively manage patients at high risk of medicines-related problems and address deviations from guidelines by developing excellent working relationships with hospital colleagues.
Conduct computer searches to identify patients at high risk of harm from medicines. Work with patients and practice teams to minimise harm through medicines optimisation.
Provide a telephone helpline for patients with questions and concerns about their medicines. Hold clinics for face-to-face clinical medicines reviews, focusing on tackling inappropriate polypharmacy. Undertake clinical medication reviews and reconciliation with patients with multi-morbidity and polypharmacy, implementing prescribing changes and ordering relevant monitoring tests.
Make recommendations to GPs for medicine improvements and support multidisciplinary teams.
Support public health campaigns and provide specialist knowledge on all public health programmes available to the public.
Answer all medicine-related enquiries from GPs, practice staff, and patients.
Manage drug availability issues and medication queries proactively by liaising with community and hospital pharmacy colleagues. Develop effective working relationships.
Conduct clinical audits and improvement projects, collaborating with colleagues such as GP trainees, practice managers, and nurses.
Contribute to developing guidelines and procedures for GP practices across the Primary Care Network and negotiate their adoption within practices.
Person Specification
Qualifications (Essential)
* Relevant pharmacy degree with current registration with the General Pharmaceutical Council
* Independent prescribing qualification
Experience (Essential)
* Minimum 2 years post-registration experience
* Sound clinical and therapeutic knowledge and evidence-based healthcare understanding
Desirable
* Clinical Diploma
* Membership of the Primary Care Pharmacy Association
* Membership of the Royal Pharmaceutical Society
* Understanding of General Practice and prescribing strategies
* Experience within Primary Care or community health teams
