Hiring: Care Coordination Manager
Location: Wokingham (Office-Based)
Salary: £40,000 – £45,000 DOE
We're recruiting for an experienced Care Coordination Manager to lead and oversee a high-performing coordination function within a growing complex care provider. This is a pivotal operational leadership role, responsible for workload allocation, rota governance, package mobilisation, and stakeholder engagement across complex and lower-risk care packages.

In this role, you will:
Lead and manage a team of Care Coordinators, allocating workload across complex and lower-risk packages
Oversee rota planning, workload balancing, and SLA-driven task management
Ensure proactive scheduling, forward planning, and visibility of care delivery
Act as the main scheduling contact for new package mobilisation and transitions
Work closely with recruitment, clinical, and operations teams to match carers to packages
Monitor compliance with care records, data entry, and documentation standards
Represent the coordination function at MDT meetings and liaise with commissioners and stakeholders
Track and report key operational metrics including rota gaps, mobilisation timelines, and team performance
Conduct performance reviews, coaching, and team development
Identify process improvements and lead service optimisation initiatives
Participate in the on-call rota on a rotational basis
What we're looking for:
• Minimum 4 years' experience in care coordination, operations, or workforce planning
• Proven people management and leadership experience
• Strong knowledge of rostering, payroll, and care management systems
• Excellent organisational, communication, and stakeholder engagement skills
• Calm, solution-focused approach under pressure
• Full UK driving licence
Desirable:
• NVQ/QCF Level 3+ in Health & Social Care or Management
• Understanding of CQC regulations and complex care delivery
