Job Opportunity: Advanced Practitioner – Early Intervention Adults Location: BRE, Bucknalls Lane, Bricket Wood, Watford, WD25 9NH Contract Type: Full-Time, Permanent Working Pattern: Office-based with opportunity for hybrid working Salary: 34LTD or 27 PAYE Start Date: ASAP Make a Difference Every Day – Join Our Early Intervention Team We are seeking an experienced and motivated Advanced Practitioner to lead our Early Intervention Vehicle (EIV) team based at BRE, Bricket Wood, Watford. This exciting and rewarding role involves urgent community-based work to help individuals remain safe and supported in their homes, avoiding unnecessary hospital admissions. Working within an integrated Health and Social Care setting, you will supervise and support a team of Community Care Officers (CCOs), deliver complex assessments and interventions, and promote community-based solutions through a person-centred approach. What You’ll Do: Lead and support a team of CCOs delivering urgent community-based assessments Complete and oversee Care Act-compliant assessments and interventions to support individuals at risk of hospital admission Supervise and develop social care staff and promote best practice across the team Work collaboratively with health professionals, local authorities, voluntary and private sector organisations Support unpaid carers and ensure culturally sensitive services are offered to all service users Drive innovation, peer development, and ensure statutory health and social care responsibilities are met to a high standard About You: Essential: Qualified Social Worker or Occupational Therapist with at least 2 years post-qualifying experience Current registration with Social Work England or HCPC Full UK Driving Licence and access to your own vehicle Excellent understanding of The Care Act, safeguarding principles, and Hertfordshire’s Connected Lives model Experience or willingness to undertake training in a specialist area, such as: Moving and Handling Practitioner Practice Educator Approved Mental Health Practitioner (AMHP) Best Interest Assessor (BIA) Desirable: Experience working in urgent response or integrated care teams Previous supervision or leadership experience within health or social care Why Join Us?
