Opportunity within the Suffolk Stroke Early Supported Discharge Team for a Band 3 Rehab Assistant role.
Main duties of the job
* Work within a stroke interdisciplinary Early Supported Discharge team operating 7 days a week, 09:00 to 17:00, providing patient service.
* Provide individually tailored rehabilitation programmes under the direction of Therapists (Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology) and collaborate with partnership agencies such as Dietetics, Nursing, patients' Suffolk GP, and Stroke Consultant.
* Deliver therapeutic intervention to patients following a stroke in their own home/communities, enabling optimal independence and maximum rehabilitation potential.
* Work collaboratively across the Integrated Stroke Service to ensure seamless provision of services across acute hospital, inpatient rehabilitation, and Early Supported Discharge teams.
* Contribute to the assessment, implementation, and evaluation of evidence‑based treatment programmes for patients with a range of impairments and complex health needs.
* Contribute to service development and data collection within the Integrated Stroke Services.
* Work in a variety of settings including patients' homes, care homes, community centres, and clinics.
Person Specification
Qualifications
* GSCEs in English grade A‑C or equivalent
* NVQ level II in care/rehabilitation or equivalent
* NVQ level III in care/rehabilitation or equivalent
* Equivalent care related study
* Care Certificate
Experience
* Experience of working within a care or rehab setting
* Previous community experience
* Previous experience as a generic worker or rehab assistant within a multi‑disciplinary team
* Experience of working within a stroke rehabilitation environment
Knowledge
* Ability to work independently and as part of a team
* IT knowledge
* Evidence of own development through PDP
* Knowledge of the role of other members of the multi‑disciplinary team
Skills
* Ability to make own travelling arrangements to meet the needs of the service
* Availability to work Monday to Friday to meet the needs of the service
