The Team works closely with other disciplines such as nursing and mental health teams to provide a multi disciplinary and integrated service in a beautiful but remote and rural setting.This is an exciting opportunity for a qualified social worker to work in a community care context with adults.We are a small and motivated Team that work hard to support adults with care needs in Skye and Lochalsh. Good Team working skills are essential as we work closely to support each other in meeting the diverse range of challenges we faceRoles include working with older adults, adults with learning disabilities, autism and those in transition from Children’s servicesDuties will include:Supporting vulnerable adults and their families and carersWorking with clients with a person centred approachWorking within relevant legislation including the Adult Support and Protection, Adults with Incapacity and Mental Health ActsWorking with other NHS professionals to provide a multi-disciplinary framework to interventionsAssessment and report writingArranging, with the client’s consent, care packages to meet assessed needWorking as part multi-disciplinary service which supports adults living within a large remote and rural geographical area with the potential challenges this can present.Flexible working is encouraged and an application for part time or jobshare would also be considered.
