Position Details
Job Title: PCN Pharmacist
Location: E16 Health
Salary: Up to £45,000 depending on experience
Hours: 37.5 (Monday to Friday 09:00‑17:00)
Responsibilities
* Collaborate closely with clinicians to enhance patient care and make cost‑effective prescribing decisions.
* Run daily telephone triage clinics and assess patients clinically where indicated.
* Review and triage pathology results and contact patients as necessary.
* Provide practice‑based review and clinical assessment for patients with long‑term conditions or complex needs.
* Reconcile medicines on hospital correspondence and liaise with local pharmacies to improve outcomes.
* Lead the practice repeat prescription service and manage queries from reception staff and patients.
* Participate in home visits and immunisation initiatives to improve service quality.
Qualifications & Experience
* GPhC registered.
* Excellent verbal and written communication skills.
* Ability to work well as part of a team and willingness to develop clinically.
* Can‑do attitude and genuine desire to improve patient care.
