As a leading care provider, we have an opportunity for a Care Coordinator in the Liverpool Branch, supporting a company that truly values its staff.
Main duties and responsibilities
1. Work with frontline staff to ensure they understand the hierarchy of the organisation and the levels of accountability, including their own.
2. Nurture a culture of respect and compassion for our customers, their carers, and families within the frontline team delivering care to underpin high standards of service delivery.
3. Promote a positive working environment that nurtures and rewards good practice through personal professional development.
4. Manage the current staff team efficiently, providing support to customers, and ensuring good relationships within the team. Report any issues to the Care Manager.
5. Provide supervision in accordance with procedures to ensure compliance standards are met.
6. Monitor staff performance, including absenteeism, and report concerns to the Care Manager, working together to address disciplinary issues.
7. Identify and implement corrective measures when individual performance does not meet standards, reporting as necessary.
8. Maintain effective communication with frontline staff, informing them of any changes to caseloads or schedules.
9. Ensure care workers receive timely and accurate information to provide safe and effective care as per individual care plans.
10. Promptly accept new referrals to prevent delays in service provision.
11. Review geographical workforce capacity to optimise frontline resources.
About you …
We are seeking warm, charismatic, and friendly individuals who can support care staff in making a difference to our clients' lives.
A caring individual committed to improving client care and supporting care staff.
Ideally, you will have a driving license and access to a car, with previous experience in domiciliary care and coordinator roles.
The successful candidate should possess a Level 3 qualification in Health and Social Care or be willing to obtain it.
This role involves 40 hours per week, with a mix of early and late shifts, plus 2:4 weekend and on-call duties as per the rota.
