St John's Medical Centre has an exciting opportunity for a Practice Nurse to join our enthusiastic and supportive Practice Team. The successful candidate must be organised, motivated and caring to ensure the highest levels of care are delivered to our patient population through patient assessments, diagnosis and clinical management.

St John's Medical Centre is a large, fast paced GP surgery located in Grantham, offering excellent transport links with easy assess to the A1 and on the mainline for London. Two notable Grammar schools in Grantham are Kesteven and Grantham Girls School and The Kings School. We currently have circa 13,000 patients registered at the Surgery and retained our 'Good' rating in our latest CQC Regulatory Review.

The role is for 30 hours a week and includes working: Monday, Tuesday, Wednesday and Friday. Pay is dependant on experience.


Main duties of the job

To be responsible for the deliveryof practice nursing services, working as part of the practicesmultidisciplinary team who provide care within their scope of practice to theentitled patient population. The practice nurse will be responsible for a numberof clinical areas such as health promotion, chronic disease management, healthprevention, well woman and well man clinics, as well as supporting themanagement team in the reviewing of clinical policy and procedure.

In our last CQC Inspection we retained our 'Good' rating.

Job responsibilities

PrimaryDuties and Areas of Responsibility

* To liaise with the registered GP andother practice based staff in addition to all other providers and servicesutilising, where appropriate a multi-disciplinary approach
* To implement and review individualcare plans, self-management plans ensuring preventative measures are detailedand agree trigger thresholds to contact the surgery
* Carrying out home-visits to patientswhere an urgent clinical need has been identified including those esculated byEMAS or a member of the Neighbourhood Team
* Liaise with primary, secondary andspecialist care services as required
* Work with the Neighbourhood Team tohelp identify patients at risk of loss of independence or admission to hospitalas a result of inadequate social support
* Act as a point of contact between GP,Neighbourhood Team, people and their carers
* Ensure care plans are communicated toGPs and other members of the Neighbourhood Team involved in the patients care
* Maintain accurate and up to daterecords of patients consultations using relevant software e.g Systm One
* Complete relevant health checks inline with patient needs
* The ability to work alone- whenundertaking home visits/ work in the community
* Develop, implement and embed healthpromotion and well-being programmes
* Implement and evaluate individualtreatment plans for chronic disease patients
* Identify, manage and support patientsat risk of developing long-term conditions, preventing adverse effects onpatients health
* Provide routine nursing care topatients as required in accordance with clinical-based evidence, NICE and theNSF
* Provide wound care (ulcer/Doppleretc.) to patients
* Request pathology services asnecessary
* Process pathology results as required
* Provide chronic disease clinics,delivering patient care as necessary, referring patients tosecondary/specialist care as required
* Maintain accurate clinical records inconjunction with extant legislation
* Ensure read codes are used effectively
* Maintain chronic disease registers
* Develop, implement and embed wellwoman clinics
* Develop, implement and embed well manclinics
* Chaperone patients where necessary
* Assist GPs with minor surgery whenrequired
* Prioritise health issues and interveneappropriately
* Support the team in dealing withclinical emergencies
* Recognise, assess and refer patientspresenting with mental health needs
* Implement vaccination programmes foradults and children
* Support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice)
* Liaise with external services/agenciesto ensure the patient is supported appropriately (vulnerable patients etc.)
* Delegate clinical responsibilitiesappropriately (ensuring safe practice and that the task is within the scope ofpractice of the individual)
* Support the clinical team with allsafeguarding matters, in accordance with local and national policies
* Understand practice and local policiesfor substance abuse and addictive behaviour, referring patients appropriately


Person Specification


Experience

* Experience of working in a primary care environment
* Experience of working as a practice nurse or community nurse
* Experience of chronic disease management


Qualifications

* Registered Nurse
* Post graduate diploma or degree (Primary Care)


Requirements

* Flexibility to cover annual leave and sickness when required.
