Main duties of the job
As a Specialist Community Public Health Nurse, you will possess excellent communication skills and broad knowledge of working in the community with children and their families.
You will be a solution-focused team player supporting the delivery of the universal offer in a 0-19 integrated neighbourhood team.
You will manage a universal plus and partnership plus caseload, working collaboratively with other agencies to ensure protection and the best health outcomes for children and families in Bolton. You will work within the community, dealing with clients from diverse backgrounds and varying needs.
Specialist Community Public Health Nurses in Bolton deliver an evidence-based service using a wide range of assessment and intervention tools, providing excellent support alongside a wider multi-disciplinary 0-19 service.

Job responsibilities
Specialist community public health nurses work autonomously in Bolton, leading on the delivery of The Healthy Child Programme. This includes health and development reviews, health promotion, screening, and support for parents, carers, children, and young people. The programme assesses strengths, needs, and risks, providing opportunities to discuss concerns and aspirations. Growth and development are monitored at key points to promote health and wellbeing and facilitate early detection and management of health needs. Additional support is provided at critical times to protect children and families at risk of harm. The model emphasizes universal services with additional contacts for those with identified needs.
