Offer description

Overview

At Change Grow Live, we believe in empowering people to change the direction of their lives, grow as individuals, and live life to its full potential.

This is an exciting opportunity to join Change Grow Live as an Assertive Outreach Worker, supporting people who are not currently accessing treatment services. You’ll play a key role in reaching and engaging some of the most vulnerable and hard-to-reach individuals in the community — including those experiencing homelessness, mental health challenges, or entrenched substance use.

Using assertive and creative outreach approaches, you will help service users move towards recovery and stability, while also developing strong partnerships with local agencies to create seamless pathways into treatment. If you're passionate about inclusion, skilled in building trust, and ready to make a meaningful impact, we’d love to hear from you.

This is a dynamic and rewarding role where no two days are the same. You’ll be working directly with some of the most vulnerable and hard-to-reach members of the community, including those who may be experiencing homelessness, mental health issues, or entrenched substance use.

You will also be facilitating training to outside agencies on the support we offer as a service. This role will also cover attending external events to promote our service, this may sometimes be on a weekend or evening.
