Community Diabetes Specialist Nurse

We are seeking an enthusiastic registered nurse with some experience in diabetes to join our dynamic team. The role involves clinical management of people with diabetes in a variety of community settings, including community clinics, care homes, and home settings. The community diabetes specialist nurse team has a strong commitment to delivering excellent diabetes care and knowledge.

This role is essential in providing expert care, education, and support to patients with diabetes across the community setting.

You will work autonomously and collaboratively to assess, plan, implement, and evaluate person centred care. The post holder will play a critical role in empowering individuals to manage their condition effectively, improving outcomes and quality of life.

Main duties of the job

* Deliver advanced nursing care for patients with diabetes in the community.
* Conduct holistic assessments and create individualised care plans.
* Educate patients, families, carers on diabetes self-management and prevention.
* Provide expert guidance to primary care teams and other community professionals.
* Support continuity of care through seamless coordination between services.
* Educate and upskill healthcare professionals on diabetes management and care.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Person Specification


Knowledge Skills and Training

* Membership of the relevant Professional Body
* Non-Medical Prescriber (if professionally appropriate)
* post registration diploma level specialist training and/or short courses or demonstrable extensive experience in the relevant specialty


Job Specific experience

* Experience at Practitioner Band 5 level
* Ability to prioritise and organise workload effectively
* Experience of specialist working
* Experience of managing change


Managerial/Supervirsory experience

* Experience of providing clinical supervision and mentoring to junior staff
* Experience of delivering training
* Experience of devising and delivering training


Information Technology/Resources

* Experience of using electronic patient / service user record systems
* Able to analyse data and produce reports using Microsoft Excel and Word
* Experience in using technology to facilitate education


Personal Qualities/Attributes

* Evidence of demonstrating the Trust's values and behaviours
* Able to communicate effectively at different levels of the organisation and with staff, patient/service users, visitors or external organisations both verbally and in writing in the exchange of highly complex, sensitive or contentious information which may require the use of negotiating and/or persuasive skills.
* Able to analyse and assess situations and to interpret potentially conflicting situations and determine appropriate action, where there is a range of options and judgement is required
