The post holder is responsible for the assessment, planning, implementation and evaluation of evidenced-based nursing care, working collaboratively and co-operatively with others to meet the needs of patients and their families.
Assist in the management and organisation of their clinical area and participating in the education, development and supervision of other staff members.
Main duties of the job
ØMaintain patients nursing records to ensure information is accurate and up to date
ØEnsure that effective communication is established and maintained with patients and carers/relatives,
ØCommunicate complex and sensitive information to patients/families, including patients with special needs/learning disabilities or other barriers to communication.
Provide accurate, up-to-date evidenced based care and training to patients and their families
Provide adequate training to patients and their family in a timely manner to ensure their safety in undertaking home haemodialysis independently at home

Ensure patients receive training in a timely manner to ensure they are competent to undertake HHD independently at home
Work autonomously as a lone worker in the community as per trust policy
Ensure patients receive regular contact from the team
