Overview
Community Occupational Therapist to assess and prescribe care packages for patients with long‑term conditions, frail & elderly, palliative care and rehabilitation, aiming to achieve quality of life and independence.
Responsibilities
1. Work within the integrated team to facilitate early discharge from hospital.
2. Work within the integrated team to prevent unnecessary admission to hospital.
3. Work with all health care professionals and statutory/non‑statutory agencies to provide a seamless, integrated service.
4. Undertake comprehensive occupational therapy assessments of patients, including those with complex presentations, using investigative, analytical and clinical reasoning skills, and formulate a graded treatment plan.
5. Formulate and execute individualised occupational therapy treatment plans of patients in collaboration with the integrated team, other agencies and the patient, utilising a wide range of treatment skills.
6. Provide assessment of functional ability and safety of patients in the community, to identify care, equipment, adaptation and therapy needs.
7. Assess & plan treatment, including teaching patients to utilise techniques & equipment to maximise their functional ability.
8. Be aware of Assistive Technology and make referrals for this as required.
9. Assess patients' holistic needs, communicating complex and sensitive information to patients and carers about their assessment, diagnosis, prognosis and treatment plan.
10. Communicate effectively with patients and carers to maximise rehabilitation and treatment potential, and to aid motivation and understanding throughout the treatment process.
11. Adjust explanations in the presence of barriers to understanding, such as hearing impairment or mental capacity impairment, to obtain consent and concordance.
12. Use tact and persuasive skills to gain the patient's cooperation in their treatment plan.
13. Liaise with GPs, social services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately.
14. Participate in multi‑disciplinary/multi‑agency meetings as appropriate (e.g., Gold Standard Framework).
15. Plan and prioritise workload for self and others according to service targets and needs.
16. Ensure patient‑held records are completed for each visit and that SystmOne inputting is completed daily.
17. Plan and organise specialist services within the integrated team.
18. Demonstrate dexterity and coordination when using specialist equipment and fine tools, manual and mobilising skills.
19. Order equipment and supplies via electronic ordering systems for use within the team.
20. Be aware of budget and ensure all effort is made to work within this.
21. Ensure informed consent is obtained prior to initiating interventions.
22. Adhere to infection control policies and universal precautions when exposed to bodily fluids, infected material, or blood products.
Qualifications
* B6 Occupational Therapist.
* Experienced and self‑motivated, with a positive “can‑do” attitude.
* Sound clinical reasoning skills and good communication skills.
* Ability to work independently and manage a varied caseload in a community setting.
* Familiarity with assistive technology and equipment ordering systems.
* Knowledge of collective work with multidisciplinary teams, including GPs, social services, inpatient teams, other allied health professionals and the voluntary sector.
Terms & Benefits
This is a 12‑month fixed contract covering maternity leave. The role provides ongoing personal development through in‑service training, regular appraisals, a personal development plan and clinical supervision. It is based in the areas serving Rackheath, Brundall, Wroxham, and Acle.
