Your Role: Work within the SPA - triage for hospital discharges Apply eligibility criteria under the Care Act ✅ Complete Continuing Health Care Assessments / Reviews Conduct Mental Capacity assessments in line with MCA 2005 Use your knowledge of hospital discharge pathways to support smooth transitions What We're Looking For: Experience in adult social care or hospital discharge pathways Strong knowledge of Care Act eligibility criteria and CHC processes Ability to complete Mental Capacity Assessments confidently Excellent communication and decision-making skills What happens now?
