Overview

We are looking for enthusiastic, skilled and motivated Community Healthcare Support Workers to join our Quickstart service across Wandsworth and Merton boroughs to deliver high quality, timely care support with an enablement focus to patients in the community. We have 1 positions that are permanent.

Quickstart service provides rapid response social care services for patients who are at risk of social care breakdown. Without a social care package in place these patients would otherwise be admitted to hospital due to an overriding social care need.

Quickstart works closely with Maximising Independence Team, a dynamic, multi-disciplinary team of physiotherapist, occupational therapist, speech and language therapist and rehab assistants, allowing for joint working with rehabilitation and learning opportunities. The post will provide an opportunity for the successful candidate to develop their clinical skills and knowledge base.


Responsibilities

* Deliver high quality, timely care support with an enablement focus to patients in the community as part of the Quickstart service across Wandsworth and Merton.
* Work in partnership with the Maximising Independence Team to support rehabilitation and learning opportunities.
* Provide rapid response social care services to patients at risk of social care breakdown and prevent unnecessary hospital admission.
* Contribute to joint working with rehabilitation professionals and participate in skill development opportunities.
