This is an exciting opportunity for a Complex Discharge Facilitator to join the Complex Discharge team on its mission to support the wards in delivering safe, timely and effective discharge for our patients. You will work collaboratively with the multi‑disciplinary team (MDT) to reduce long lengths of stay in the Acute Hospital. Working within the Integrated Discharge Bureau (IDB), you will provide support for patients, relatives and staff from admission to discharge, and develop strong working relationships with partners such as Adult Social Care, Central Surrey Health, Continuing Health Care, North West Surrey CCG and local councils. The post is 37.5 hours a week covering a 7‑day service (including Bank Holidays and weekends) plus one duty shift a week until 6pm. Responsibilities

* Coordinate patient discharges within the Integrated Discharge Bureau (IDB), working autonomously and engaging the MDT to identify and resolve delays.
* Provide comprehensive discharge advice and information to ward staff, patients and relatives, including guidance on funding, placements and care packages such as Action for Carers and Home Safe Plus.
* Manage data and record all discharge information for a cohort of patients, ensuring accuracy and timely documentation in IT systems (IP, PTL, Cerner).
* Support the D2A (Discharge to Assess) process through multi‑agency coordination, facilitating timely discharge for patients and families.
* Identify critical delays in the diagnostic, treatment and care processes, propose solutions, and liaise with department managers and the Team Leader for consideration.
* Cover ward areas as directed by Team Leads or Managers in the absence of other Discharge Team members.
* Attend board rounds daily, facilitating discussions around discharge and following the Trust’s Discharge Policy in liaison with community, social services and other healthcare colleagues.
* Ensure patient confidentiality is maintained, keep all documentation complete and accurate, and complete Datix incident reports as required.
* Support multi‑disciplinary meetings to resolve outstanding blocks to successful discharge and address disputes in discharge planning.
* Assist in identifying patients eligible for Continuing Health Care and support referrals through Fast Track processes.
* Provide training and support for ward staff on D2A referrals, discharge policies and safe discharge checklists on Cerner.
* Participate in audits of discharge planning effectiveness, collecting and analysing relevant data.
* Perform required administrative and rehabilitation support roles when needed.
