We have an exciting opportunity for a Release and Transfer Clinical Lead to join our Primary Care team at HMP Brixton, a category C Prison located in London.


Hours

37.5 hours per week, Monday to Friday, 08.00-16:00 or 09:00-17:00


Salary

This position is full time, you’ll receive an annual salary up to £63,119 per annum - depending on experience.

Candidates are required to have a legal right to work in the UK on a permanent basis and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.


What you'll be doing

* Hold complex case release meetings and attend prison release meetings to ensure that an adequate handover has been given
* Work alongside the Mental Health Clinical Lead to deliver a seamless service for our patients
* Implement the pathway that creates a smooth pathway for our patients back into the community or transferred to another establishment
* Oversee the management of assessments of health needs, planning, delivery and evaluation of care, for the prison population, including clinical risk assessment

Qualifications And Experience Required

* Qualified Adult Nurse with NMC PIN
* Evidence of implementation of local and national policies e.g. NICE guidance, developing protocols/care pathways and monitoring effectiveness
* Experience demonstrating high levels of clinical expertise in decision making and judgement
* Knowledge of clinical audit and its relationship to research
* Line management experience
