Job OverviewWe are seeking a dynamic Transformation and Digital Manager to work with the Operations Manager in a senior leadership role to implement neighbourhood health services, leading on the delivery of service/pathway redesign. With a focus on moving services into the neighbourhood that are proactive and/or provide preventative care, the use of population health data will be key to designing services which achieve these aims. The role will require experience in quality, service improvement and project management with expertise in transformational change and a good understanding of general practice systems and processes, including familiarity with general practice digital systems and how they can be used to deliver improvement.
The postholder will work to implement the digital transformation described NHS 10year plan to maximise digital opportunities for both SLR PCN and the neighbourhood. This post while home based, will be expected to work closely with the 9 GP practices that make up South Lincs Rural PCN.

About us
South Lincs and Rural PCN (SLRPCN ) covers a population of roughly 100,000 patients registered with 9 GP surgeries practices covering the Bourne, Spalding and Holbeach areas.
The team at SLRCN are working collaboratively to provide services in addition to those already found at your local GP practices. Primary care networks have the potential to benefit patients by offering improved access and extending the range of services available to them and by helping to integrate primary care with wider health and community services. South Lincs and Rural Primary Care Network brings together nine GP Practices, health and care partners, patient organisations and the voluntary sector across East Lindsey. Our aim is to transform patient and carer access to health and care information, services, self-help and guidance.
SLRPCN established a not for profit Limited Liability Company called Allied Health South Lincs Ltd (AHSL) to manage the financial and operational aspects of the PCN. The nine practices of the SLRPCN are the sole shareholders of AHSL.
Responsibilities
Operations and Service Improvement
Support the delivery of PCN transformation priorities in line with the Network Contract DES/Single Neighbourhood Provider contract
Assist in the development and implementation of any new/redesigned services/pathways which improve access and health outcomes for patients and are delivered closer to their homes.
Develop and implement mechanisms for assessing the impact of agreed changes.
Contribute to the development and delivery of the PCN transformation plan which will design and deliver new proactive ways of working – i.e neighbourhood MDTs, left shift of high volume elective specialties, underpinned by digital technologies
When necessary, develop clear and comprehensive project plans including
* Communication and engagement with stakeholders

Establish positive relationships with practices and within the Neighbourhood, providing hands‑on capacity and support to practices in implementing agreed new transformational projects
Collaborate with peers in other PCNs and work with the local ICU to support the implementation of overall primary care transformation strategies, including digital transformation strategies.
Advise on innovative opportunities and support practices to maximise benefits from innovation, through the sharing of learning and best practice.
Keep PCN leadership and member practices updated on progress of current initiatives, progress and issues so that plans can be adapted in response.
Understand current challenges to effective service delivery and identify solutions, including existing best practice within the PCN.
Build relationships across the PCN to develop consensus for improvements and to ensure business and clinical staff are fully engaged throughout the process of selection, implementation and deployment of change.
Link with the ICB and other partners to gain buy‑in for transformation and to support alignment with broader strategy and improvement approaches.
Digital
Develop and manage a digital transformation plan for the PCN which optimises the use of digital technology (e.g. digital triage/ambient voice technology) including identifying interdependencies, managing risks, considering the potential impacts on the wider organisation, and determining resource requirements.
Take the lead in agreed digital projects ensuring that the PCN can harness the full benefits of digital technology to support at scale service provision i.e. S1 Hub.
Support the practices to adopt, fully utilise and sustain their use of digital technology to benefit patient care (e.g. digital triage/ambient voice technology, workflow tools)
Undertake all DPIAs for new projects
Complete annual DSA/DPA/Data Security & Protection Toolkit on behalf of the PCN.
Be aware of, and take steps to reduce, digital exclusion for our PCN population.
Maintain SLR PCN & AHSL website and all social media for SLR PCN and AHSL.
Provide hands‑on capacity and support to practices in implementing and sustaining agreed new technologies and digital initiatives across the PCN
Provide on‑site support to practices for any digital/telephony issues
Represent the PCN at all system wide digital meetings
Manage access and queries for the Care Portal for existing and new staff.
Assist with any Digital/Telephony queries for the AHSL team and PCN practices.
Build relationships across the PCN and Integrated Care Board (ICB) cluster to gain buy‑in for transformation projects and to support alignment with broader strategy and improvement approaches.
Information and data management
Provide business intelligence support through the use of population health management tools (Eclipse, SHAPE, Pathfinder) to identify potential areas for service improvement/redesign or support in the development of any new patient pathways.
Work with the Operations Manager to identify relevant data sets for the PCN which use data to track and assess impact of changes made to service/operational processes over time.
Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges.
Support and encourage a focus on data in understanding the effectiveness of changes made.
Seek and secure analytical resource as required to support the PCN in developing and implementing appropriate data and information sharing mechanisms.

* Support the Operations Manager to take advantage of neighbourhood development opportunities which will benefit member practices of South Lincs Rural PCN.

* Qualifications/education
* Educated to degree level in healthcare, business, or equivalent level of knowledge and experience working at a senior level
* Master's degree level qualification in a relevant area
* Formal project management qualification (PRINCE Foundation as minimum) or equivalent training and experience
Experience
* Experience of working in general practice with a good working knowledge of general practice systems (including IT, digital and BI tools), processes, and procedures;
* Experience of successfully delivering change and improvement programmes in a patient (or customer) facing environment.
* Experience of successfully delivering IT projects/transformation and/or improvement projects in primary care
