Community Staff Nurse

Dorset HealthCare University NHS Foundation Trust is seeking a Community Staff Nurse to join our Blandford team. This full‑time, fixed‑term role is offered on secondment to NHS staff. Applicants requiring Skilled Worker sponsorship to work in the UK are ineligible.
The post operates primarily between 08:00–20:00 with some attractive weekend enhanced pay. Flexible shift patterns may be considered to fulfill service needs.
Responsibilities include:
* Delivering a high‑quality, person‑centred approach to care delivery while safeguarding safety, privacy, dignity and embodying the Trust's values.
* Undertaking assessment of patients' care needs and developing programmes of care within a multidisciplinary team.
* Managing acute and chronic nursing needs and developing strong therapeutic relationships with patients.
* Supporting people to maintain wellbeing and independence, preventing unnecessary admission to hospital.
* Delivering exceptional end‑of‑life care, including symptom management and assisting patients with preference achievement.
* Identifying care needs, monitoring conditions, recording observations and helping patients develop their wellbeing care plans.
* Coordinating with community partners to optimise health outcomes.
Requirements:
* Membership of the relevant professional body.
* Evidence of recent professional development in an up‑to‑date portfolio.
* Knowledge and understanding of trust strategy and relevant policies.
* Supervised clinical placement experience.
* Ability to mentor students or supervise junior staff.
* Demonstrable knowledge of assessment and therapeutic interventions in area of practice.
* Experience contributing to management of change.
* Recent prior experience in a comparable role.
* Experience demonstrating duties to junior staff.
* Experience providing clinical supervision and mentoring to junior staff.
* Ability to evaluate data and produce reports using Microsoft Excel and Word.
* Experience using electronic patient/service user record systems.
The role requires an Enhanced DBS check (cost covered by the Trust). Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of a new check.
