Our inpatient care and management is overseen by a matron along with a team of registered nurses, healthcare assistants, therapeutic workers, physiotherapists, occupational therapists, rehabilitation assistants, care management staff, a pharmacist and our inhouse medical team.
Community hospitals provide a range of comprehensive healthcare services for patients over the age of 18. The units support our enablement model and aim to get patients home as quickly as possible. Patients are referred to the ward either from the acute setting or from home via their GP or local community teams.
You will be a key member of the multidisciplinary team driving high-quality discharge planning.

Main duties of the job
1. Leading daily discharge plans from admission through to safe, timely discharge.
2. Co-ordinate communication with patients, families, outside agencies and the multidisciplinary team.
3. Drive and chair daily board rounds, updating expected discharge dates and actions.
4. Ensuring the RIO board round notes are accurate daily.
5. Adhere to local KPI's
6. Document discharge progression daily into RIO progress notes.
7. Identify and remove delays by liaising with therapy, pharmacy, transport and community services.
8. Escalate discharge barriers promptly to the Nurse in Charge, Matron, Flow matron or Bed Manager.

Qualifications, training and experience
Essential criteria
9. A level 5 foundation degree level of study, or will be able to demonstrate the equivalent level of experience.
10. Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C/ 4-9 or equivalent in English and Maths).
11. Recent experience as a healthcare worker gained through education, work experience or previous employment
Knowledge and skills
Essential criteria
12. You will need to be able to be able to use IT systems and databases.
13. You will fully understand the role and remit of the Discharge Coordinator.
14. You will be able to demonstrate a good level of knowledge in clinical practice.
15. Having an awareness of developments within the NHS would be an advantage.
Working environment
