What you will do:
* Manage a caseload that includes highly complex patients, providing specialist assessments within your identified area of expertise.
* Deliver therapy interventions in community settings, including crisis response services, rehabilitation wards, clients’ homes, and care homes.
* Continuously assess, implement, and evaluate individual care programmes tailored to patients’ needs.
* Collaborate with a multidisciplinary team, including consultants, GPs, nursing staff, and allied health professionals, to ensure effective patient management and successful discharges.
* Provide clinical education and training to staff within community services.
Requirements:
* HCPC registered Occupational Therapist.
* Valid UK driving licence.
* Ability to work across a variety of community settings.
* Strong assessment, rehabilitation, and multidisciplinary working skills.
