Our Strengths‑Based Model of Practice supports a multidisciplinary approach to enabling and supporting people who live with issues caused by disability, frailty, and ageing.
Team and Service Focus
They will be based within the Early Intervention and Prevention Hub, which includes the short‑term social work, community support workers and the front‑door response in Adult Social Care. The team supports a multidisiplinary approach, with a focus on adults who have multifaceted complex needs and cognitive decline. This service focuses on outcomes for individuals, maximising individual potential and minimising risk to independence, with an emphasis on choice and control.
Qualifications
A recognised social work qualification and registration with the Care Council for Wales is essential.
The ability to greet customers through the medium of Welsh is a requirement for this post.
An Enhanced Children and Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
A valid driving licence is requirement for this post.
Responsibilities
Protecting children, young people or adults at risk is a core responsibility of all council employees.
