Overview
We are seeking a dedicated and compassionate Specialist Palliative Care Nurse to join our multi-professional team, providing expert care, support, and advice to patients with palliative care needs and their carers across the Trust. You will respond to referrals, deliver specialist clinical nursing expertise tailored to patient and family needs, offer holistic care, provide clinical leadership within the specialty, and contribute to the ongoing development of palliative care services. You will also be involved in education and research initiatives and collaborate with healthcare professionals within and outside the Trust to ensure continuity and excellence in patient care.
Responsibilities
* The post holder will be responsible for the assessment of patients with specialist needs, and the development and evaluation of clinical management plans including non medical prescribing (where appropriate).
* The post holder will support Advance Care Planning, including awareness, initiation of discussions, and authorship of ReSPECT/DNACPR decisions (Policy No C09).
* Deal with complex facts or situations requiring analysis, interpretation and comparison over a range of options.
* Exchange highly sensitive, complex and emotive verbal and written information with patients, staff and carers, using differing strategies to meet individual needs and overcome barriers to understanding.
* Contribute and participate in the presentation of information to senior staff relating to performance and incident management, action plans, etc.
* Participate in clinical research, audit and trials.
* Work without direct supervision, managing time effectively, organizing own workload, and maintaining supportive networks for self and colleagues.
* Recognise own abilities and limitations.
* Contribute to the formal induction of new members of the specialist palliative care team and work with the multi-professional team to provide specialist, expert care, advice and support to patients and carers throughout the Trust.
* Respond to referrals and provide specialist clinical nursing knowledge and expertise in various aspects of palliative care; provide clinical leadership in partnership with senior clinical nurses and aid the development of education and research relevant to the specialty.
* Liaise with other healthcare professionals within and outside the Trust to ensure the best possible care and continuity of care for patients.
