Wickhambrook Surgery in Suffolk is seeking a permanent Care Coordinator to join our team and support our Primary Care Network (PCN). This part-time role offers 20 hours per week, working on Mondays, Wednesdays, and Fridays, with one evening until 6.30pm. Although supporting the PCN, the role is based at Wickhambrook Surgery.
The ideal candidate will possess excellent customer service skills, enjoy working in a busy environment, and maintain accuracy in all tasks.
Main duties of the job
* Assist with personal care plans for patients, ensuring preventative actions support appropriate service use.
* Gather and review patients' care and support needs, creating personalized plans aligned with best practices.
* Help patients manage their needs by answering queries, scheduling appointments, and providing quality information to support decision-making.
* Coordinate and navigate patient care across health and social care services, collaborating with social prescribing link workers, health and wellbeing coaches, and primary care professionals.
* Support the delivery of enhanced services and other PCN requirements.
* Contribute to achieving QOF, incentive schemes, QIPP, and other quality or cost-efficiency initiatives.
* Assist patients in accessing self-management education, peer support, and health interventions to improve wellbeing and activation levels.
* Run regular patient searches using the clinical IT system to monitor progress towards key performance indicators.
* Collaborate with the practice/PCN data manager for annual patient recall systems.

West Suffolk Rural PCN comprises three GP surgeries—Wickhambrook, Glemsford, and Guildhall Clare—serving approximately 15,800 patients. PCNs are a national NHS initiative aimed at delivering more integrated local health and care services.
Wickhambrook Surgery is a friendly, thriving rural practice with three GP partners, a salaried GP, GP registrars, nurses, HCAs, and a dedicated support team. We are a dispensing practice committed to providing high-quality patient care. Our location is centrally situated in the countryside, with good road access but limited bus links, so own transport is essential. The role is based at Wickhambrook Surgery.
