Job Summary
An opportunity has arisen for a permanent position within the Tissue Viability Clinical Nurse Specialist Team to support the Primary and Community Care services and Secondary Care Services.
The Tissue Viability Team is a small team of expert Nurses working closely with wider multi-disciplinary teams to promote a collaborative culture for individualised wound care for all patients.
The post holder will work as an integral member of the Tissue Viability Team. They will provide an expert professional and clinical contribution to the nursing care and management of patients with complex wounds. The post holder will facilitate relevant practice development in line with National standards, guidance and best practice through clinical practice, leadership, education, research, and audit which reflect the corporate clinical governance objectives.

Main duties of the job
The Successful Applicant Will:
1. Work as an autonomous practitioner.
2. Accept referrals from all health care professionals both in primary and secondary care in accordance within set protocols for referral.
3. Provide a tissue viability nurse service for patients with wounds in both Primary and Community services and Secondary Care settings.
4. Be responsible for managing a defined caseload.
5. Assess tissue viability problems of patients and initiate appropriate treatment & investigations, interpreting, acting on and communicating the results appropriately to the patient.
6. Use expertise and clinical skills to assess patients' needs, educate, and give practical advice on treatment management.
7. Recommend prescriptions and advise on treatments as per nurse prescribing legislation and protocols.
8. Promote understanding and knowledge for patients in managing their conditions and instruct on maintaining healthy lifestyles.
9. Acquire special skills and techniques under supervision, such as Recording Ankle Brachial Pressure Index, working within policies and protocols.
10. Assist in developing nursing staff skills within Primary, Community Care, and Secondary Care settings.
