An exciting opportunity has arisen for the Adult Community Nursing Service. We are looking to appoint a Community Matron to work 3 days per week, passionate about high-quality patient-centered care, to join our highly regarded Community Matron Team. The service operates from 7am to 7pm across the Twickenham borough. The post is advertised as a 6-month fixed-term contract to cover a secondment within the team, for 22.5 hours per week.
The post holder will provide line management and leadership for a locality nursing team, being accountable for the team caseload and the development of a skilled, competent, and motivated workforce capable of meeting patients' needs.

Key Responsibilities:
* Deliver the locality nursing service in line with the commissioned service specification, standards, activity, and outcomes, within a multi-disciplinary locality working framework.
* Provide line management and leadership for a locality nursing team, ensuring the team is skilled, competent, and motivated to meet patient needs.
* Work collaboratively with primary healthcare teams, social services, and the wider multidisciplinary locality team to assess health needs holistically and provide nursing care to housebound patients with various medical needs and long-term conditions.
