Highly Specialist Cardiac Pharmacist (Supporting Primary Care)
Role: 37.5 hours a week, including back‑up on call and weekend commitments.

An exciting opportunity has arisen for a motivated and enthusiastic pharmacist to join the cardiac team at Barts Heart Centre within Barts Health NHS Trust.
The post holder will work alongside the cardiovascular prevention primary care pharmacy team and deliver quality care to patients through efficient medicines optimisation. The post provides several opportunities for professional and personal development, contributing to quality improvement projects and supporting the delivery of world‑class and innovative cardiovascular services.
Key responsibilities include:
* Co‑ordinating closely with the cardiovascular prevention primary care pharmacy team and wider primary care MDT (GPs, pharmacists, nurses, practice managers, community pharmacy) to plan workload and priorities.
* Optimising cardiovascular medicine for long‑term conditions related to cardiac disease across primary care services within North‑East London (NEL) and beyond.
* Working with primary care teams to continually develop the service, maintaining high standards and achieving best practice to meet the needs of patients and the Trust.
* Communicating effectively with patients to help optimise their pharmaceutical management and liaising with other health care professionals to design, implement plans to optimise health outcomes.
