Barrack Lane Medical Centre has an opportunity for a care navigator to join our friendly, supportive, and professional team on a full-time basis, 37.5 hours per week. Although you will be employed by the Barrack Lane and Ivry Street PCN, you will be based at Barrack Lane.
Responsibilities include:
1. Greeting and directing all patients, handing out prescriptions, letters, completed forms, and specimen bottles.
2. Making appointments face-to-face, online, and via the telephone.
3. Answering the telephone promptly and politely.
4. Managing patient needs, dealing with inquiries, and advising on practice procedures.
5. Helping patients access the appropriate services.
6. Ensuring all information is accurately recorded on the computer system.
7. Working as part of the care navigator team and supporting all practice staff members.
The care navigator role is vital within the surgery as they are often the first point of contact for patients. The environment is challenging and requires a calm, caring approach, with the ability to work under pressure. The ideal candidate will be a highly motivated team player with customer service/patient experience and excellent communication skills. Previous applicants need not apply.
Barrack Lane is a busy practice located in the town centre of Ipswich, Suffolk, serving over 20,000 patients across two sites: the main surgery and a branch at Stoke Park. The practice is part of a Primary Care Network (PCN) with Ivry Street Medical Centre, covering a population of 32,000 patients. The PCN enables resource sharing and offers enhanced services, including care coordinators and social prescribers to assist vulnerable patients in accessing the best care.
