Overview
The post involves working with people with learning disabilities who may additionally have a variety of mental and physical health conditions, facilitating activities both on and offsite. The aim as part of a multi-disciplinary team is to help patients develop meaningful activities to support their wellbeing both whilst in hospital and help develop skills and insights into their health together with coping strategies for them to use when discharged from hospital. To provide day to day care for patients under supervision of a registered nurse which adheres to best practice, standards and guidelines, and contributes towards the patients care and treatment.
Responsibilities
* Work with people with learning disabilities who may have mental and physical health conditions to facilitate activities on and offsite.
* Collaborate as part of a multi-disciplinary team to support patients in developing meaningful activities and coping strategies for use after discharge.
* Provide day-to-day care for patients under the supervision of a registered nurse, adhering to best practice, standards and guidelines.
Qualifications and Skills
* Desirable: ability to speak Welsh; Welsh and/or English speakers are welcome to apply.
