Job Summary

* The ability to work autonomously and as part of a team.

Main Duties
The post holder will be responsible, alongside two other CNSs, for patients with chronic kidney disease within community clinics across Leeds and Mid-Yorkshire. Responsibilities include managing anaemia in these patients, with opportunities to develop a coordinated anaemia management service across renal pathways. The role involves independent assessment, care planning, treatment, and evaluation within protocols, facilitated by Consultant Nephrologists. Additionally, the post involves developing educational programs for practice nurses in primary care.

Leeds Renal Services, part of the Abdominal Medicine Surgery (AMS) Clinical Service at Leeds Teaching Hospitals NHS Trust, manages adult kidney disorders, ranging from mild to life-threatening. The main renal unit is at St James's University Hospital, offering inpatient, outpatient, pre-dialysis, dialysis, and transplant services, including nurse-led clinics.
