The Crawley Healthy and First Community Primary Care Network Team is looking for a registered nurse to join the team. We are based at Crawley Hospital, covering Crawley and Horley.

Any successful applicant will have an individual induction plan to ease into this community environment.
Responsibilities
* Diabetic management
* Wound management, including leg ulcer diagnosis and treatment
* Palliative care
* Bladder and bowel care, including urethral and supra‑pubic catheters and other continence needs
* Partnership with local health and social providers serving a specific group of people under frailty
* Support patients in rehabilitation, self‑management and independence throughout the continuum of care
* Participate in a team of colleagues supporting evidence‑based care and facilitating staff learning and development
