This role involves working closely with the Senior Patient Safety Advisor, Patient Safety Manager, Head of Clinical Governance, and Deputy Chief Medical Officers to identify and manage incidents. The appointee will participate in weekly meetings to discuss learning response options under the Patient Safety Incident Response Framework (PSIRF). Additionally, the role includes collaborating with colleagues across the Trust to promote a culture of open reporting and investigation, supporting divisional governance teams and Trust managers, and assisting in developing and delivering training programs. The appointee will review learning outcomes to identify systemic issues and ensure appropriate actions are taken to prevent recurrence and facilitate shared learning across the Trust. The position reports to the Senior Patient Safety Advisor.

Main duties of the job
* Evaluate the impact of incidents.
* Coordinate and manage the weekly Safety, Learning & Improvement Conversation (SLIC), including:
* Identifying incidents for review
* Drafting agendas for SLIC meetings
* Managing meeting administration, including creating agendas, collating papers, taking minutes, and preparing weekly learning slides
* Attending Patient Safety Response meetings approximately twice a week and documenting outcomes
* Maintaining investigation logs
* Collaborating with Legal Services, Complaints, Tissue Viability, Information Governance, and Safeguarding to ensure effective communication regarding incidents and investigations
* Participating in weekly meetings to review legal cases, safeguarding issues, complaints, and patient feedback
* Communicating relevant aspects with local Health & Safety teams, including incident reporting and disease regulations
* Liaising with the Assurance Team, Integrated Care Board, and NHS England to share documents and respond to FOI requests
* Providing evidence and maintaining an audit trail for Duty of Candour compliance

Job responsibilities
* Support Divisions in investigating patient safety incidents according to Trust policy, reviewing and approving investigation reports and action plans.
* Monitor progress of investigations and ensure deadlines are met.
* Maintain investigation files and relevant correspondence.
* Update the Ulysses incident reporting system with investigation information.
* Assist in drafting and reviewing local standard operating procedures for the Patient Safety Team.
* Edit and format investigation reports for internal and external sharing, including with patients and families.
* Manage weekly global Safety Message emails.
* Maintain professional development and stay informed on relevant legislation, standards, and guidance.
* Work within ethical standards and legal requirements, including GDPR, Caldicott Guidelines, and the Human Rights Act.
* Undertake additional duties as reasonably required.

Skills and Experience
* Excellent oral and written communication skills
* Strong interpersonal skills
* Ability to manage disagreements and conflicts effectively
* Experience working with colleagues of various seniority levels
* Proficiency in Microsoft Office
* Understanding of NHS structure, processes, and governmental healthcare organizations (CQC, NHS England, MHRA, NICE, HSSIB)
* Knowledge of PSIRF
* Specialist risk qualification
* Experience in patient safety and NHS environments
* Data analysis skills
* Knowledge of clinical governance issues such as audits and mortality reviews
* Training in human factors, after-action reviews, or PSIRF investigations
