Job overview
The Specialist Community Public Health Nurse for Inclusion will support the development of pathways based on specific geographies and communities within Darlington to reduce health inequalities and improve health outcomes for vulnerable communities.
Main duties of the job
The post will ensure service delivery reflects demographics, existing locality infrastructure, and feedback influences service improvement. The SCPHN for Inclusion, alongside other practitioners within the Darlington 0-19 Service, will be responsible for expanding outreach to those experiencing the greatest inequalities. The 0-19 Service will focus on early intervention and prevention, supporting a reduction in health inequalities and improved health outcomes.

* Asylum Seekers, refugees, or vulnerable migrants
