Nursing Associate with Children & Young People

Are you a compassionate and motivated Nursing Associate looking to make a real difference to children, young people and their families? We are seeking an enthusiastic Nursing Associate to join our dedicated Childrens Community Nursing Team across Warwickshire. This is an exciting opportunity to work within a supportive multidisciplinary team providing high-quality care to children and young people with acute, complex and long-term health needs in their homes, schools and community settings.
About the Role
As a Nursing Associate within the Childrens Community Nursing Team, you will play a key role in delivering person-centred care and supporting children and young people to remain at home whenever possible. Working under the direction of registered nurses, you will contribute to assessments, care planning and the delivery of clinical interventions while promoting the health, wellbeing and independence of children and their families. The role offers a varied and rewarding workload, including supporting children with complex health needs, undertaking delegated clinical procedures and working closely with families, schools and partner agencies.
Main duties of the job
The Nursing Associate has a breadth of knowledge across the lifespan and across the fields of nursing, providing holistic and person-centred care and support for people of all ages and in a variety of settings. The Nursing Associate works independently under the leadership of registered nurses, working within the sphere of nursing and care and within all aspects of the nursing process.
Nursing Associates are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to employment as a nursing associate and will work to a nationally recognised code of conduct.
The following criteria identify the core components of the roles, responsibilities and accountabilities of Nursing Associates. They identify what Nursing Associates can contribute to the health and wellbeing of patients and service users and apply across all health and care settings. Integral to all of these is the ability to communicate effectively, with sensitivity and compassion, and to manage relationships with people, making reasonable adjustments where necessary.
Nursing Associates will be required to advocate for Children and Young People (CYP) referred to Childrens Community Nursing Team (CCNT). The post holder will be required to support at times with Clinical Education, Palliative Care, Generic CCN work and to support with caseload management of Children known to Continuing Care
Job responsibilities

* Work in partnership with the CYP and their family delivering holistic care with the support of a named nurse
* Participate in the review of individual healthcare plans in association with the named nurse, CYP and family
* Work flexibly over a 24-hour period, 365 days a year, including nights, weekends and bank holiday shifts
* Meet travel requirements of the post, working within a wide range of locations across the County of Warwickshire and having independent means of transport
Promoting Health and Preventing Ill Health
* Support CYP to improve and maintain their mental, physical, behavioural health and wellbeing where possible dependent on the trajectory of the CYPs condition
* Actively involve in the prevention of and protection against disease and ill health
* Engage in public health, community development, and in the reduction of health inequalities
* Support health and social care colleagues with completing assessment of CYP
Provide and Monitor Care
* Provide compassionate, safe and effective care and support to CYP in a range of care settings
* Undertake training and complete competencies for specific nursing skills in order to facilitate care provision and assessment
* Monitor the condition and health needs of CYP within their care on a continual basis in partnership with people, families, and carers
* Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment
* Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care
* Recognise and report any situations, behaviours or errors that could result in poor care outcomes
* Use a computer or networked system, responsibly ensuring data is collected and entered accurately and timely, and complying with all South Warwickshire NHS Foundation Trust policies including the Lone working policy
Working in Teams
* Play an active role as a member of interdisciplinary teams
* Collaborate and communicate effectively with nurses and the wider MDT, including professionals from social care and education
* Support in the supervision of Nursing Associate students on placement with the CCNT
Improving Safety and Quality of Care
* Improve the quality of care by contributing to the continuous monitoring of CYP and families experience of care
* Identify risks to safety or experience and take appropriate action, putting the best interests, needs and preferences of CYP first
* Participate in effective and comprehensive audits and measure against benchmarked targets set within the framework
* Maintain the medical equipment database, ensuring all equipment is serviced as per SWFT policy
* Contribute to policy reviews
Contributing to Integrated Care
* Contribute to the provision of care for CYP, including those with complex needs
* Understand the roles of a range of professionals and carers from other organisations and settings who may be participating in the care of a CYP and their family
* Understand responsibilities in relation to communication and collaboration
Application
* GCSE level A - C English and Maths or equivalent
* Nursing Associate Foundation Degree/Higher Apprenticeship
Experience
* Significant experience in healthcare
* Experience as part of a team
* Minimum 6 months experience supporting Children and Young People
* Experience of community palliative care
Skills
* Able to implement and evaluate treatment, contributing to or completing assessments as required
* Experience supporting Children and Young People with complex healthcare and technological needs
* Able to recognise when escalation to a registered professional is required
* Sound understanding of Child protection and safeguarding processes
* Able to manage time effectively
* Demonstrates knowledge of research-based practices
* Demonstrates knowledge of clinical audit and clinical effectiveness
* Good written, verbal and non-verbal communication skillsAble to present information, written and orally in an accurate clear and logical manner
* Able to write reports about an assessment of an individual
* Ability to organise and prioritise workload
* Ability to write accurate and legible patient notes
* Good spoken and written English
