Overview
We are seeking a compassionate and dedicated Care Manager to oversee the delivery of high-quality care services within our facility. The ideal candidate will possess extensive experience in senior care, particularly in Home Care environments. This role requires strong leadership skills and the ability to develop and implement effective care plans tailored to meet the individual needs of residents, including those with dementia and Alzheimer's.
Duties
* Supervise and manage care staff, ensuring adherence to best practices in senior care.
* Develop, implement, and review personalised care plans for clients, focusing on their unique needs and preferences.
* Provide direct support in medication administration and oversee compliance with healthcare regulations.
* Foster a safe and nurturing environment for clients, promoting their dignity and independence.
* Collaborate with healthcare professionals, families, and external agencies to coordinate comprehensive care services.
* Conduct regular assessments of client's health and wellbeing, adjusting care plans as necessary.
* Lead training sessions for staff on dementia care, Alzheimer's care, and other relevant topics to enhance service delivery.
* Maintain accurate records of client's care plans and staff performance.
Requirements
* Minimum SVQ 3 Qualifications.
* Proven experience in Homecare settings is essential.
* Experience in developing care plans tailored to individual needs.
* Demonstrated management skills with experience supervising staff in a Homecare environment.
* Knowledge of dementia and Alzheimer's care practices is highly desirable.
* Strong leadership abilities with excellent communication skills to liaise effectively with clients, families, and team members.
