Job Overview
This is a permanent position to meet the needs of the service. The postholder will be directly accountable to the caseload holder and will be responsible for assessing, planning, implementing, and evaluating nursing care in the community. The role requires collaboration with the District Nursing Team and other agencies to provide clinically effective care. Welsh language skills are desirable; applications from Welsh and/or English speakers are equally welcome.

We offer developmental and training opportunities and are seeking motivated, passionate Band 5 Community Nurses.
The successful candidate will have excellent communication skills and the ability to provide clinically effective care within the District Nurse Team. The West Division of Betsi Cadwaladr University Health Board delivers District Nursing Services across Gwynedd and Mon counties, working with partner organizations to deliver high-quality, evidence-based care aimed at maintaining patients in their homes, reducing hospital admissions, empowering self-management, and ensuring seamless, patient-centered care.
You must be able to travel between patients' homes in towns and rural areas promptly. This position operates on a rotational 24-hour schedule.
