We have an exciting opportunity in Intermediate Care in South Manchester. Pathway 2 includes rehabilitation beds at Buccleuch Lodge Intermediate Care Unit in West Didsbury and supporting patients in their home environment on our 'Home Pathway'.

Buccleuch Lodge provides inpatient care to 14 patients, providing rehabilitation with the aim of returning patients home with support. We have a unique community setting with dedicated nursing and therapy staff.

The rehabilitation beds are accessible for both step down from a hospital setting or step up patients from home.

The teams integrated service delivery model aims to avoid admissions, reduce length of stay and improve the patient's experiences through access to the right intervention at the right time, delivered by the appropriate health or social care practitioner.

We currently have a full time vacancy for a B3 Therapy support worker who is compassionate, forward thinking and a team player that wants to progress within the environment of community rehabilitation.

After completing appropriate competencies, you will be expected to manage your own caseload of patients, both in our bed based unit and patient's own home.

The role will also involve team/office based duties to ensure effective service delivery.

The team is committed to continual professional development with regular supervision and appraisals. This post is supported by experienced B4 rehabilitation support workers, Physiotherapists and Occupational Therapists.

Applicants must have a UK driving license and access to a vehicle to be able to carry out home visits and between the bases are per the job requirements. Please note, evidence of this will be required. If you are unable to provide this evidence yo will not progress to interview.
