Our Myton at Home Service provides palliative and end of life care in the community.
An exciting opportunity has arisen for an experienced Registered Nurse to join our Myton at Home team.
You'll provide direct clinical care, clinical leadership and coordination of the Myton at Home Service.
You will be acting as a role model and specialist practitioner, having both an advisory and clinical input and focus into patient care.
You will need to be flexible and able to travel across the South Warwickshire and Rugby areas.

Main duties of the job
You will provide physical, psychological, social and spiritual care for patients and carers.
You will liaise and work collaboratively with all staff in The Myton Hospices, and external NHS organisations to ensure the patient and their carer receive safe and high-quality care.
You will undertake holistic palliative care assessments and identifying a management plan covering all identified needs of the patient and their families in conjunction with the community key worker.
You will monitor and assess symptoms and facilitate effective symptom relief.
About you
We would love to hear from Registered Nurses with:
* Current NMC registration
* Degree in Nursing or equivalent
* Post-registration experience in palliative / end-of-life care
* Clean UK driving licence (this role requires driving)
* The right to work in the UK (we cannot support with sponsorship)
If appointed, we'll arrange an DBS check including the adults' and children's barred lists because you will be providing healthcare including in patients' homes.

Job responsibilities
Role Purpose
Reporting to the Deputy Manager of the Myton at Home Service, the post holder will be responsible for theassessment, planning, implementation and evaluation of individualised holisticcare.
To contribute to the clinical day-to-day coordination, leadership and management of the Myton at Home service. This is a patient-focused, clinically-based role which manages and processes referrals, liaises closely with and communicates effectively with referrers. The care and support provided is for people in the last weeks and days of life, who wish to remain at home.
It includes individual holistic patient assessments including symptom management, advice, referring patients for Inpatient beds where appropriate, supporting discharges from the hospice and hospitals, and advance care planning. The role is to facilitate and maintain the philosophy of the hospice and a palliative care approach. To ensure that the Myton values are upheld at all times. The post-holder will at all times practise with care, compassion, competence, good communication, courage and commitment.
Main Duties & Responsibilities
* Act as a role model and specialist practitioner, having both an advisory and clinical input and focus into patient care.
* Undertakeholistic palliative care assessments and identifying a management plan coveringall identified needs of the patient and their families in conjunction with thecommunity key worker (Clinical Nurse Specialist, GP, District Nurses).
* Providephysical, psychological, social and spiritual care for patients and carers.
* Monitorand assess symptoms and facilitate effective symptom relief by a) reporting promptly to the community key worker and b) administering symptom relief medication as per prescription, policy and in line with community guidance and Myton policies and procedures.
* Work collaboratively,promote and maintain effective relationships with multi-professional teams andother care providers; have a proactive approach in supporting discharges whereappropriate, in liaison with the Discharge Coordinator / hospital dischargeteam.
* Actas a positive role model to others and ensure that practice of specialistpalliative care is evidence based and promoted within the clinical area.
* Duringtimes of need, work in the Inpatient Units when required.
* Be proactivein preparing and delivering clinical information to promote the effectivenessof multidisciplinary team meetings, acting as a key member to ensureeffective communication.
* Ensure that all clinicaldocumentation is completed correctly and appropriately in line with NMCGuidance.
* Promote and maintain a culture of evidence-basedclinical practice, ensuring the creation of effective local networks to sharegood practice and dissemination.
* Tobe aware of the political and national influences affecting clinical care, suchas NICE guidance / End of Life Care Strategy.

* Upholdand ensure compliance with the organisation's policies and procedures, ensuringall staff are clear in relation to their roles and responsibilities for RiskManagement, Health and Safety, Infection Prevention & Control, SafeguardingAdults & Children, Data Protection, and other relevant policies.
* Participatein the development and evaluation of policies / guidelines / procedures forclinical practice, responding to changes in service demands, and undertake clinicalaudit as required by the organisational requirements and the ClinicalGovernance agenda.

* Practicean approach that facilitates and fosters learning for carers and professionals (students, junior staff and those on placement to Myton), sharing expertiseappropriately and supporting each learner to achieve their learning needs.
* With the support of the Clinical Education team, assessand monitor the competence and confidence of team members around all aspects ofclinical practice, identifying learning needs, supporting, and supervising thedevelopment of knowledge, skills and confidence.
* Beresponsible for own continued professional development, maintaining up to date evidence-based knowledge, including an awareness ofthe national and local influences affecting palliative and end of life care.
* Ensureown compliance with mandatory training and professional developmentrequirements, maintaining records of training and development undertaken forself, in preparation for NMC revalidation.
