Overview

We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker who will deliver a range of quality care services to patients visited by the Community Health Support Team based with the Community Response Hub at Tarporley Hospital. The team can visit patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.

Travel across the Western Cheshire footprint is essential for work purposes, so transport for work MUST be demonstrated. Applicants MUST demonstrate how they will meet the travel requirement for this post. End of life care experience is required to be considered for this post.


Hours and Salary

The hours are part time 19 hours per week, consisting of 2 x 9.5 hour shifts, worked on a rota basis between 08:30 – 14:00 and 18:00 – 22:00, including Bank holidays and weekends.

The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.


Responsibilities

As part of a friendly team, you should have experience of working in patients\' homes and the ability to carry out tasks such as:

* Simple dressings
* Catheter care
* Personal care
* Recording blood sugars
* Care for palliative patients

This post involves Lone working as well as working as a member of a team. Travel across the Western Cheshire footprint is essential for work purposes so transport for work MUST be demonstrated.
