We are recruiting for a full-time Assistant Care Co-ordinator to support care provision and patient flow in our Intermediate Care Service in Cambridge.
You will work across the county-wide Intermediate Care Service, primarily supporting colleagues based in our South teams. You will be able to work flexible shift patterns within our core service hours, including early and late shifts, covering weekends and bank holidays.
The vision of the service is to provide outstanding, patient-focused, community-based healthcare and therapy, supporting earlier discharge from hospital as part of the Home First model.
Main duties of the job
* Assist the Care Co-ordinators within the Intermediate Care Service, ensuring referrals are managed safely and promptly.
* Coordinate and organize care packages delivered by the local Intermediate Care Service to support community-dwelling patients.
* Collaborate closely with the Team Lead and senior therapy staff to promote a multidisciplinary approach to patient care.
* Work with colleagues across social care, health, and the third sector to provide integrated, patient-centered care.
