What you will do
* Provide outcome-focused assessments for adults facing physical disabilities or age-related challenges, helping them reclaim their independence.
* Collaborate seamlessly with teams to ensure effective hospital discharge processes and other key services.
* Offer knowledgeable guidance and support to individuals, helping them overcome daily challenges and improve their quality of life.
* Utilise the Mosaic case management system to record and monitor progress, refining your technical skills in the process.
* Travel as needed (must have a valid driving license and vehicle) to meet clients and deliver on-site support and guidance.
Requirements of the Occupational Therapist
* Degree or equivalent in Occupational Therapy
* Recent experience working as an Occupational Therapist in the UK
* Track record of NHS and local authority experience
* Registered with the HCPC
* Experience in hospital discharge services is essential
* Expertise in the Mosaic case management system is desirable
