Overview
The Hospice at Home service cares for patients across Cambridgeshire who choose to remain in their own home in their last weeks of life. We are holistic in our approach and the care is tailored to each person’s specific needs. The compassionate team of Nurses and Healthcare Assistants provides practical and holistic care supporting patients and their families in their homes.
Band 6 Community Sister / Charge Nurse – permanent, part time (32 hours per week) role at our hospice in Cambridge, covering Cambridge City, South Cambridgeshire and Ely.
Responsibilities
* Provide expert knowledge and skills in the provision of palliative and end-of-life care within the Hospice at Home service.
* Mentor and line manage Band 2, 3 and 5 colleagues.
* Facilitate holistic assessment, treatment and evaluation of patient care; coordinate and manage Healthcare Assistant teams; ensure cover across the service.
* Work flexibly across the community palliative caseload with the Community Palliative Clinical Nurse Specialist and assess patients at home if required.
* Ensure high standards of care and documentation, and promote best practices within the team.
Requirements
* Be an NMC Registered Nurse with a relevant postgraduate qualification (or equivalent experience) and evidence of continuing professional development.
* Previous post-registration experience at Band 5 and knowledge of palliative and end-of-life care.
* Right to work in the UK; driving licence and access to a vehicle for work purposes (required).
* Enhanced DBS with Adult and Children’s Barred List checks.
Additional Information
* Starting salary: £38,682; progression to £46,580
* Pro-rata for part-time roles; salary negotiated with consideration for qualifications and strengths
