Band 5 Staff Nurse - Ward 15 and Ward 28

Are you an enthusiastic and motivated nurse with a passion for caring older people?
We're looking for a dedicated Band 5 Staff Nurse to join our friendly and supportive ward team, caring for frail older adults, many of whom are living with dementia or complex health needs.
There are a number of full‑time and part‑time positions available on:
* Ward 15 - Healthcare for older people & frailty
* Ward 28 - Stroke
Main duties of the job
As a Band 5 Staff Nurse, you will play a key role in delivering high‑quality, patient‑centred care to older adults and stroke patients with complex physical and cognitive needs.
You will be responsible for assessing, planning, implementing, and evaluating evidence‑based nursing care, supporting patients through their recovery journey, and promoting independence and dignity at every stage. Working closely with doctors, therapists, and support staff, you will contribute to holistic rehabilitation, discharge planning, and patient education.

Person Specification
Qualifications
* Evidence of professional and clinical knowledge acquired through training to degree/diploma level leading to Professional registration on the appropriate part of the NMC register i.e. (Adult / Child / Learning Disability or Mental Health)
* Evidence of undergraduate education for newly qualified nurses / or post graduate continuing professional development for registered nurses who have post registration experience
Experience
* Previous / recent experience or working in an acute secondary health and care setting (can include undergraduate student placements within the last 12 months)
* Willingness to become a practice supervisor / assessor to or meets Nursing and Midwifery Council (NMC) standards for student supervision and assessment
* Demonstrable understanding and knowledge of the NMC code of conduct and the proficiencies and standards for registered nurses can apply this to undergraduate training or post graduate practice
* Being an accountable professional
* Promoting health and preventing ill health
* Assessing needs and planning care
* Providing and evaluating care
* Leading and managing nursing care and working in teams — including prioritisation and delegation skills
* Coordinating care and responding to deteriorating healthImproving safety and quality of care
* Awareness of current relevant clinical and professional issues and NHS policy
* Understanding / experience of the principles of Safeguarding and Mental Capacity
* Awareness of Health and Safety issues.
* Experience in working in an Acute NHS practice setting as an undergraduate student nurse or post graduate registered nurse.
Additional Criteria
* Demonstrable communication and relationship management skills which include
* Listening – writing – reading – speaking in English with understanding of contemporaneous record keeping
* Good IT skills ability to record information electronically
* Competent in a range of clinical skills/ nursing procedures willingness to undertake expanded practices relevant to the scope of practice require
* Competent in managing the care of a group of patients / shift management (take charge of the practice setting in the absence of more senior staff) relevant to the practise setting.

* Displays genuine interest in the post applied for

* Ability to work a varied shift pattern including nights / weekends unsocial hours where required
* The applicant can describe why the Trust may deploy him / her to a different practice setting for a span of duty.
