We are looking for a Band 5 community nurse who can be responsible for delivering a high standard of nursing care to patients in a community setting. This is a full‑time 12‑month fixed‑term role.


Main duties of the job

The successful candidate will be able to work independently, assessing and providing care to individuals with various health needs. This will include those with chronic illnesses, post‑hospitalisation care, wound management, and palliative care. The role involves collaborating with other healthcare professionals and the wider MDT, supporting patient recovery, and promoting self‑care in negotiation with patients and their carers and families.


Job responsibilities

Job Purpose

* To provide nursing care to a variety of patients in their own home.
* To work as part of the integrated health and social care teams in the community, providing holistic care to patients. This nursing care includes acute and chronic wound management, medication administration, catheter care, palliative care, long‑term conditions and self‑care.
* To provide holistic and individualised care to patients and carers as working autonomously and as part of a team.
* To regularly take responsibility for care and workload management in the absence of the Team Leader or deputy.

Dimensions

* To provide nursing care for house‑bound patients who are registered with a Nottingham City GP.
* To participate actively in the services training strategy, including mandatory training and essential training and any other training if identified necessary for this role.
* To participate in continuing professional development as relevant to maintain NMC registration requirements.
* To work across the Nottingham City Community Nursing Teams as required.
* To supervise and support student nurses on placements with the community nursing teams.
* To supervise unregistered nurses who have been delegated patient care.

* Plan, deliver and evaluate programmes of care to address peoples health and well‑being needs.
* Deliver evidence‑based care.
* Seek patient and carer feedback about the service.
* Deliver a high standard of care to patients and carers.
* Contribute to the continual improvement of the service.
* Maintain professional standards as outlined in the NMC code.
* Aware of the Clinical Governance agenda and apply relevant risk management systems.
* Participate in relevant research and audit activity.
* Regularly take responsibility for the appropriate delegation of nursing care to team members.
* Promote and maintain record‑taking practice in line with the CityCare and NMC and professional standards.
* Liaise with individuals and groups in a wide variety of settings and situations.
* Communicate and escalate as required.
* Exhibit good communication and team working skills.
* Be responsible for the development of own knowledge and clinical practice and recognise one's own limitations.
* Act as a mentor to a variety of nursing students.
* Participate in the development of other team members.
* Participate in the supervision framework as a supervisee and supervisor.
* Contribute to the implementation of service developments.
* Engage in the personal development process and take responsibility for reaching agreed development objectives.
* Attend mandatory training.
* Utilise enhanced knowledge and skills to promote continuous service improvements.
* Promote and maintain best practice that ensures the health, safety and security of self, patients and colleagues in accordance with the policies, procedures and guidelines of NCCP.
* Practice in accordance with safeguarding legislation and guidelines.
* In the absence of the District Nurse Team Leader or deputy, take responsibility for the needs of patients and carers on the community nursing caseload and ensure that appropriate delegation and supervision of work is carried out in accordance with the skills and abilities of the team.


Special requirements

* Ability to be flexible over hours worked within contracted hours to meet the needs of the service.
* Ability to work out of hours.
* Full driving licence and the ability to travel between locations.
* To participate in and fulfil the requirements of the Directors on‑call arrangements.


Knowledge and skills

* Up‑to‑date clinical knowledge including wound care, management of continence and palliative care.
* Understanding of equality and diversity.
* Effective communication and interpersonal skills.
* Basic IT skills.
* Able to work independently recognising own limitations.
* Motivated and resourceful.
* Time‑management and organisation skills.
* Able to work under pressure.
* Positive approach to change.
* Management of continence and palliative care related to working in the community.
* Able to work unsupervised.
* Able to motivate others.


Experience

* Up‑to‑date clinical knowledge including wound care, management of continence and palliative care.
* Postgraduate experience in a primary care setting.
* Either post‑registration experience or completion of a community management placement.
* Evidence of working in a team.


Qualifications

* Active nursing qualification with the NMC.
* Post‑registration qualifications relevant to community nursing.
