What You Will Do:
* Carry out comprehensive mental health assessments and brief interventions for service users.
* Provide short-term and long-term keyworking, supporting individuals under the Care Programme Approach (CPA).
* Manage a complex caseload, ensuring high-quality and person-centred service delivery.
* Contribute to the duty rota, ensuring seamless team operations and effective crisis response.
* Collaborate with health professionals, housing services, and community partners to deliver integrated and holistic care.
* Maintain accurate and timely case records in line with statutory and organisational standards.
