The WISHH Primary Care Network (PCN) is seeking anenthusiastic Advanced Care Practitioner, paramedic, or newly qualified GP or tojoin our PCN as part of the Additional Roles Reimbursement Scheme (ARRS). This is a fantastic opportunity to experience the diversityof primary care within the WISHH PCN, working on a full or part time basis.
Salary will be commensurate withexperience, and in line with the guidance set out in the NHS England NetworkContract DES Contract Specification 2025/26.
The successful application will join a dedicated Housebound Visiting Team. One member of our team also undertakes a weekly care home ward round, thus supporting some of our most frail and medically complex patients.
Main duties of the job
* Primarily, visiting patients at home and makinga clinical assessment of their acute and/or chronic medical problems. Actioningfurther management accordingly (arranging bloods or other investigations,referrals, prescribing as indicated, or admission if necessary).
* Other clinical duties may occasionally includeface to face consultations within the surgery, telephone consultations andqueries, visiting patients at home, prescription administration and management,general administration (referrals, pathology, letters/communications,correspondence) in a timely fashion, ensuring high quality care.
* Making decisions in relation to presentingmedical problems, whether self-referred or referred from other healthcareworkers within the organisation.
* Assessing the healthcare needs of patients withundifferentiated and undiagnosed problems.
* Screening patients for disease risk factors andearly signs of illness.
* Developing care plans for health in consultationwith patients and in line with current practice disease management protocols.
* Providing health education to patients.
* Contribute to optimise PCN Investment and ImpactFund (quality improvement markers)
* Recording clear consultation notes to generalmedical practice standards.
* Collecting data for audit purposes and complyingwith QOF requirements.
* Compiling and issuing computer-generated acuteand repeat prescriptions.

About us
Our PCN includes two practices, comprising Idle MedicalPractice and the Saltaire and Windhill Medical Partnership. Our total PCNpatient population is approximately 37000, with Saltaire and Windhill makingup two thirds and Idle one third of this cohort.
We are a forward thinking and innovative PCN and embrace thechallenges of the ever-changing primary care landscape. With the use ofeffective IT systems, collaborative working, and a strong focus on educationand learning, we deliver safe and high-quality care for our patients. We offera number of specialist services in-house, including dermatology, diabetes, MSKas well as others, and further opportunities are available for clinical supportand development. We are a friendly and supportive team, and tailored mentoringand clinical supervision will be provided for the successful applicant.
Our PCN Team itself comprises a Clinical Director (GPpartner); PCN Manager; two ACPs; two care coordinators and an administrator.Regular team meetings take place including a weekly PACT meeting, and a monthly team education and support meeting. Additionally,monthly education taking place within both practices, as well as provision forannual mandatory training, are available.
Our PCN was one of the very first to develop a dedicatedPACTeam (Proactive Care Team), undertaking proactive reviews of houseboundpatients and interfacing with other professionals such as physiotherapists,occupational therapists and social prescribers.

Person Specification
Experience
* Experience of dealing with a range of clinical conditions
* Experience of providing home visits and or working in the community with housebound patients
* Experience of working in a GP practice
* Experience of System one
* Experience in triage (digital or telephone) (the process of quickly examining/assessing patients to decide which ones are the most seriously ill and must be treated first)
Qualifications
* For a Newly qualified GP:
* -within 2 years of completion of specialist GP training; AND
* - hold full GMC registration and license to practice
Knowledge and skills
* Proven ability to evaluate the safety and effectiveness of own clinical practice
* Understanding of evidence-based practice
* Evidence of working autonomously, with some support and supervision
* Ability to effectively triage, assess and diagnose
* Requesting pathology tests and processing the results, advising patients accordingly
* Assessing and managing acute deteriorations/exacerbations of Long-Term Conditions
* In the case of an ACP/paramedic, have appropriate radiology training to enable requesting and interpreting of imaging
