An opportunity has become available within our busy reception at Well Close Medical Group to join us as a Patient Care Coordinator.
We are seeking an experienced individual with excellent interpersonal, customer service, and organisational skills who enjoys working in a fast-paced environment.
Working 26.25 hours per week, alternate weeks 8am - 1pm / 1pm - 6.30pm, Monday to Friday.
Main duties of the job
Regular duties include dealing with telephone, face-to-face, and electronic enquiries, booking and/or amending patient appointments and home visits, contacting patients to provide information from the clinical team, and assisting patients to access our services and those available in the wider community.
This role is ideal for someone who is highly organised and an effective communicator, capable of providing a professional and warm welcome to our patients, ensuring every contact is of the highest quality.
IT skills are essential. You will also process prescription requests and provide a full administrative service.
Previous reception and/or customer service experience and knowledge of SystemOne are desirable, but full training will be provided.

Job responsibilities
Reception
* Receiving patients and consulting with practice team members
* Processing personal and telephone requests for appointments, telephone consultations, and ensuring callers are directed to the appropriate healthcare professional
* Handling new patient registration forms
* Taking messages and passing on information
* Initiating contact with and responding to requests from patients, team members, and healthcare providers
* Ensuring the system is operational at the start of each day and handing over to the next shift; ensuring the answer phone is operational at the end of each day
General Administration
* Opening and locking up practice premises, maintaining security
* Processing and distributing mail
* Filing and retrieving paperwork
* Data entry and collation
* Providing clerical support such as word processing, photocopying, and scanning
* Maintaining the tidiness of the reception area
* Covering sickness and annual leave, including some weekends
* Performing additional duties as requested by management
* Undertaking mandatory training as required
Appointment System Management
* Managing bookings, edits, and cancellations
* Monitoring system effectiveness and reporting issues
* Handling home visit requests and arranging transport for hospital appointments
Medical Records Management
* Filing correspondence, reports, and results correctly
* Processing repeat prescriptions according to guidelines
* Checking patient details during prescription handover

* Confidentiality: Maintaining patient and practice confidentiality in accordance with Data Protection Act and other relevant legislation
